| Creating a New Form Template in Microsoft Word |
- Download the New Form Template document.
TIP: To download, right-click on the link and select Save Target As from the shortcut menu. Save it to your Windows Desktop or My Documents folder for easy access.
- Launch Microsoft Word and open the document that you downloaded in Step 1.
Click on View > Toolbars > Forms to display the Forms toolbar, as shown at the right
- You can update the form by copying and pasting existing rows and adding new ones with your own form fields. See below for further details on customizing the form.

When you are finished making changes to the form, click the padlock icon to turn on the form protection.

Click on File > Save As

In the File Name field, type the name of your new form

In the Save As Type field, select Document Template (*.dot) from the drop-down list

Click Save
THINGS TO REMEMBER
- The form documents have been set up in a table format.
- To add a new row to the form, select the row below where you wish to add one. Right-click and select Insert Row to add a blank row above your selection.
- To copy a row, select the row. Right-click and select Copy. Move your cursor to the beginning of the row where you want to insert it, right-click and select Paste. The copied row will be inserted above your cursor.
- To delete a row, select the row. Right-click and select Delete Row.
- To add a form field, position your cursor and click the appropriate form field from the Forms toolbar:

Text Form Field |
Use this button to insert a fill-in field where users can enter text. You can also specify a default entry so that the user does not have to type an entry except to change their response |

Check Box Form Field |
Use this button to insert a check box next to an independent option that you select or clear. Please note that these check boxed are not mutually exclusive — that is, the user can select more than one |

Drop-Down Form Field |
Use this button to insert a drop-down list box that restricts available choices to those you specify. After inserting the field, double-click on it to open the window where you can add the options which
will appear in your list. |
- Always click the padlock icon
to turn on the form protection before saving it. This will enable users to simply press TAB to move to each field and type their information.
